Credit Card Authorization

Credit card authorization for non-domestic purchases will require the following documentation: A signed credit card agreement, a facsimile of the credit card, and a copy of a photo identification displaying the address we are shipping the product to.

Agreement:

I agree to pay for the merchandise I have ordered via the internet/fax including shipping & handling fees we have agreed upon.

Credit Card Information:

We will also need to see your credit card. It can be done by taking a photocopy, or scanning the credit card image (both sides).

Photo Identification:

We need to see a photocopy identification which contains your signature, and your address (the one we are shipping products to).
